**Teen-Week Optional Self-Defense Class**

Teen-Week ( girls ages 13-18 ) will be offering an optional self-defense class for all teen-week dancers. This class is completely optional and not a required class for Glorify. Glorify is offering teen-week dancers transportation to the self-defense class from camp, if parents choose this option. This is optional class is offered completely FREE to all dancers participating in Teen-Week. This class will be held at NOVA jujitsu in Ronceverte, WV from 3:00 pm - 4:00 pm.  If your Teen Week dancer wishes to participate in our women's self-defense class, please follow this link to complete her liability waiver for NOVA. Please note, your daughter is not required to attend the self-defense class. If she wishes to not participate, you can pick her up prior to us leaving. The form below (consent to travel) is required to be filled out by all girls participating in Teen Week. If your daughter is participating in Teen Week the form below and the two forms above are required.

Please select one option below on the consent to travel form. If your daughter will be traveling with a Glorify Volunteer, if you will provide transportation, or if she will not be participating in the class.
